We look forward to working with you and getting you back to doing the things you love, PAIN FREE! Below you will find a link to our new patient intake forms, along with some information on what to expect on your first visit, as well as any follow-up treatments.

Your First Visit

Your initial appointment will take about 60 minutes. We do run on-time with our scheduled appointments, so please be sure to arrive 5-10 minutes prior to your scheduled appointment in order to fill out any additional forms that may be necessary. When you arrive you will be greeted by one of our friendly staff members and they will provide any additional forms for you to sign while they collect your insurance information.  

Next, you will be taken to your providers treatment room for your initial evaluation.  Your doctor will conduct a detailed examination.  This examination will vary based on your individual needs, but may include:

  • A detailed health history

  • A specific history of your primary concern(s)

  • Movement screening

  • Specific orthopedic and neurological testing

  • Referral for imaging if medically necessary (X-rays/MRI)

Based on the subjective and objective findings in your evaluation, your doctor will then make a recommendation for proper management of your condition, inside or outside our office. Your doctor will inform you as to what treatments are recommended, frequency of treatment, and an expected time-frame for full recovery. As always, if you have any questions or concerns, we encourage you to bring those up at this, or any time, in order that we may provide you with the best possible care.

After you have agreed to the recommended course of action, you will then receive your first treatment.

Follow-up Visits

Regular treatment visits will last approximately 30 minutes.  During these appointments your provider will begin by asking for any updates on the status of your condition. Treatments vary based on your condition and what factors have led to this dysfunction. We always ensure that patients are comfortable with any/all treatments that will be performed, and ultimately it is always the patients decision if they would like to opt out of a specific treatment modality. Wellness Care

Many people find maintenance care to be helpful for injury prevention and/or performance enhancement.  We never pressure patients to continue or begin this type of care and there is no long-term commitments.  For patients who wish to pursue this type of care, we typically recommend scheduling an appointment once every 4-6 weeks based on your activity level. Treatments may be “whole-body” if you present pain-free, or can be more specific if you have an area that has become tight or problematic.  Your provider will discuss this with you at the beginning of each visit.
